What is Fibroid Treatment through High Intensity Focused Ultrasound ( HIFU)?

High Intensity Focused Ultrasound ( HIFU) is the latest innovation in the non-invasive treatment of fibroids, enabling you to preserve fertility and have children. High Intensity Focused Ultrasound ( HIFU) combines the technologies of radiation-free MRI and ultrasound to treat the symptoms of uterine fibroids without any incisions or cuts whatsoever to the body.

How exactly is fibroid treatment done through High Intensity Focused Ultrasound ( HIFU) in India

3D MRI images are used to identify the exact location and size of the fibroid. Ultrasound energy is then delivered to the exact site of the fibroid without affecting tissue through which it passes as well as the surrounding tissue. The focused ultrasound beam generates heat of up to 65°C within the fibroid, thus destroying the fibroid cells. These destroyed cells are removed by the body naturally over a period of time, leading to a reduction in the size of the fibroid and consequent relief from the painful symptoms of fibroid.

The ultrasound energy is delivered in bursts of 15 seconds each. Each such burst of energy is called a Pulse or Sonication. Approximately 50 such Pulses are delivered over a period of 3 hours. During the entire process, the radiologist is able to monitor the progress of the treatment as well as the heat being generated at the fibroid site on a real-time basis using Magnetic Resonance Guidance (MRg). Using thermal mapping, the treatment temperature is kept in a band, such that your safety is ensured while the fibroid is being ablated. Motion sensors are also used so that the ultrasound beam is adjusted or stopped if you move on the scanning table.

You will receive a mild sedative before the procedure to keep you comfortable, but will at all times be completely conscious. You may feel slight warmth near the fibroid site during the procedure. You may also experience a slight pain or fever for up to 72 hours after the procedure, especially if a large volume of fibroid has been ablated.

Are there any situations in which this procedure is not advisable?

High Intensity Focused Ultrasound ( HIFU) is not advised in those cases where the fibroid is located very close to bowel loops, spine nerves,etc. It may also not be advisable for a few patients who have scar tissue or adhesions from earlier surgery.

Is Fibroid Treatment through High Intensity Focused Ultrasound ( HIFU) in India appropriate for fibroids of all sizes?

High Intensity Focused Ultrasound ( HIFU) can be used to treat fibroids larger than 3 cm in size. Treatment can generally be completed in one day in the case of small fibroids. In the case of large fibroids the treatment may extend to two days.

Robotic Myomectomy ( Robotic Surgery for Fibroid Removal) in India using da Vinci Surgical Robot

Our associate hospitals in India are amongst the very few in Asia which offer Robotic Surgeries (Robotic Myomectomy) for the removal of fibroids. During robotic surgery in India, the surgeon sits at the console at which she can view magnified 3-D images of the uterus of the patient. The surgeon has foot and joystick-like hand controls which she uses to ' perform' the surgery. The hand and wrist movements of the surgeon are picked up by the robot and replicated without error but on a smaller scale. The robot eliminates tremor effects from the surgeon's hands and allows minute movements during delicate phases of the surgery. The high resolution camera clearly displays the presence of small blood vessels which can be spared. Greater reach and dexterity is made possible because of the 360 degree turning flexibility of the wrist of the da Vinci Robot. Uterine Artery Embolization in India

Uterine Artery Embolization is a relatively new method for the treatment of fibroids. Uterine Artery Embolization is commonly performed at leading hospitals in India. A catheter (a thin tube) is inserted through an artery in the groin region and threaded close to the site of the fibroid. The arteries supplying blood to the fibroid are identified. Synthetic substances are then used to block the flow of blood from the artery to the fibroid. The fibroid is starved of blood and progressively shrinks in size, leading to elimination/reduction of the symptoms caused by the fibroid.

Fibroids on the inner wall of the uterus can be removed using hysteroscopy, provided that the fibroids are not deep rooted. A surgical hysteroscope, along with a viewing instrument, is introduced through the vagina into the uterus. The fibroids are removed through the vagina, leaving no external scars whatsoever. This procedure can even be done on outpatient basis with no hospital stay.

Laparoscopy Myomectomy ( Fibroids Removal Surgery) in India

Laparoscopy is a good option for removing fibroids that are growing outside of the uterus and if the number of fibroids is 5 or less. Also, ideally, the fibroids should not exceed approximately 8 cm (3 inches) in size, although larger fibroids can also be removed laparascopically.

Laparoscopic myomectomy requires a few tiny incisions in the abdomen and the use of a laparoscope to remove fibroids. A camera is introduced through one incision, while miniature surgical instruments are introduced through the others. The cutting away of the fibroids and the evacuation of the mass is performed through these small incisions. In the case of relatively large fibroids an electric morcellator may be used to cut the fibroid tissue into long strips.

Investigation - cum - Minor Treatment Procedures

Dilation and Curettage (D&C)

The lining of the uterus is scraped using a metallic device called a curette and tissue is checked for any cell abnormalities. This procedure helps to identify the cause of excessive or abnormal uterine bleeding. It is also done as a follow-up investigative procedure if cancer is detected following a biopsy or if abnormal cells are found on the pap smears of post menopausal women. Small tumours and any material left behind after an incomplete miscarriage or abortion can also be removed using this procedure

Cervical Conization / Cone Biopsy Procedure in India

This procedure is used to extract tissue for biopsy purposes, if a cervical biopsy or colposcopy have not yielded the desired results. In this procedure, a cone shaped mass of tissue is removed from the cervix. This procedure can also be a curative procedure if diseased tissue is removed completely.

Hysteroscopy in India

Colposcopy in India

Is performed if the gynaecologist observes any abnormality during an examination of the cervix or if a Pap test shows an abnormality.
