What is it?

In Acute lymphoblastic leukaemia there is an overproduction of immature white blood cells. These cells crowd the bone marrow, preventing the production of normal blood cells. The body is thus unable to prevent or fight infection. Also, inadequate numbers of red cells and platelets are made by the marrow – this causes anaemia, and easy bleeding and bruising.

Diagnosis

Acute lymphoblastic leukaemia is diagnosed by a full blood count (FBC), and a bone marrow biopsy and examination.

Treatment

A diagnosis of acute leukaemia can require immediate and intensive treatment.

For a child with Acute lymphoblastic leukaemia, the treatment period will be around two years for girls and three years for boys. In almost all cases, treatment can be started in India and continued in the patient’s home country.

The type of treatment used will depend on a number of factors including the sub-type of Acute lymphoblastic leukaemia, the genetic make-up of the leukaemic cells, and the age and general health of the person.

Chemotherapy

is the main form of treatment for Acute lymphoblastic leukaemia. A combination of drugs, including steroids, is given in several cycles with a gap of a few weeks in between. The first aim of treatment is to destroy leukaemic cells and induce a remission ( which means that there is no evidence of cancer cells in the blood and bone marrow and blood counts are normal). Once remission is achieved, further treatment is needed to help destroy any residual disease, and to prevent leukaemia from coming back.

Cancer cells can collect in the brain and spinal cord. To prevent and treat disease in this area, chemotherapy is injected directly into the fluid that surrounds these structures. Sometimes, the area is also treated using radiotherapy.

In some cases, a stem cell transplant ( Bone Marrow Transplant or BMT) may be used to treat relapsed acute lymphoblastic leukaemia, or where there is a high likelihood that Acute lymphoblastic leukaemia will relapse in the future.
